Spring Showcase Sign-Up!

Please use this form to register all participants in your family. There is a $30 registration fee for each participant that helps defer the cost of the recital. **A special, personalized outfit and 2 tickets are included in the registration fee. If you’d like to donate time or supplies the day of the show, please let us know! We can’t wait to see our students’ confidence grow as they dance and tumble in front of their loved ones!

This agreement releases GroovinKids LLC, Katy Cryns, and all related entities, from all liability relating to injuries that may occur to the student(s) listed above during dance or gymnastics instruction. By signing this agreement, I agree to hold GroovinKids LLC, Katy Cryns, and all related entities, entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence. I also acknowledge the risks involved in dance and gymnastics. These include, but are not limited to, bruises, scrapes, and muscle injury. I swear that my child is participating voluntarily, and that all risks have been made clear to me. Additionally, my child does not have any conditions that will increase his/her likelihood of experiencing injuries while engaging in this activity. By signing below, I forfeit all right to bring a suit against GroovinKids, Katy Cryns, and all related entities, for any reason. In return, I will receive dance and/or gymnastics instruction. I will also make every effort to obey safety precautions as listed in writing and explained to me verbally. I will ask for clarification when needed.

GroovinKids has put in place preventative measures to reduce the spread of COVID-19; however, GroovinKids cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending GroovinKids classes could increase your risk and your child(ren)’s risk of contracting COVID-19.
